DISCLOSURE OF DIRECTORY INFORMATION
The items listed below are designated as "Directory Information" and may be released in accordance with the Family Educational Rights and Privacy Act (FERPA) of 1974, as amended. Under the provisions of the Family Educational Rights and Privacy Act, as amended, you have the right to withhold the disclosure of "Directory Information."
Should you decide to withhold directory information, you may authorize at a later date on a transaction-by-transaction basis the release of directory or non-directory information (for example, the release of a transcript for employment purposes) or you may cancel the withhold directory. (See the form below for instructions)
The following is considered "Directory Information" at Fairleigh Dickinson University and may be made available to the general public unless the student requests non-disclosure of directory information from the Office of Enrollment Services and Records in person or in writing. If a data is not defined as “directory information” then it may not be released without the express written permission of the student or by using one of the exceptions for release permitted by FERPA
· Student's name
· telephone numbers
· major and minor fields of study
· college of major and year (freshman, sophomore, etc.)
· enrollment status (full-time/part-time) including credit hours
· dates of attendance
· degree sought and time
· degrees conferred, awards and honors conferred
· participation in officially recognized activities and sports
· weight and height of members of athletic teams
· the most recent previous educational agency or institution attended by the student
· fraternity and/or sorority and educational societies
Preventing Disclosure of Your Directory Information by the University
To keep your directory information completely confidential except for those limited circumstances, commonly known as exceptions, permitted under FERPA, please complete the form below and return it to the Office of Enrollment Services at the appropriate address. Submitting this form will make your directory information confidential until you make a written request to the Office of Enrollment Services to lift this restriction. Please be advised that requesting full confidentiality of your Directory Information will make this information unavailable to prospective employers, insurance companies, and others to whom you may want this information known or verified. For example, the University would be unable to verify degree, major or enrollment for possible employment, credit card applications, insurance purposes, mortgage information, apartment leases, etc.Thus it is recommended that students carefully consider whether personal privacy concerns outweigh the potential inconvenience of having directory information withheld.
The institution will honor your request to withhold the information listed below but cannot assume responsibility to contact you for subsequent permission to release that information. Regardless of the effect upon you, the institution assumes no liability for honoring your instructions that such information be withheld
